ANA (Antinuclear Antibody)

Gateway test for autoimmunity; positive ANA indicates potential autoimmune disease
ANA is present in many autoimmune diseases (lupus, Sjögren's, scleroderma, mixed connective tissue disease). Positive ANA indicates activated B cells attacking nuclear components. Not all ANA-positive people develop clinical disease, but it identifies increased risk.
Target Negative (< 1:80)
Suggested Action Plan

If ANA is positive, order more specific antibody tests (anti-dsDNA, anti-Sm, anti-SSA, anti-SSB) to narrow the diagnosis. Begin anti-inflammatory interventions regardless of further testing.

hs-CRP

Measures systemic inflammation; elevated in active autoimmunity
hs-CRP is elevated in active autoimmune disease and reflects systemic inflammation. Monitoring hs-CRP over time indicates disease activity and response to treatment.
Target < 1.0 mg/L
Suggested Action Plan

Lower hs-CRP through anti-inflammatory diet (Mediterranean, elimination of food sensitivities), omega-3 supplementation, regular exercise, stress management. Retest every 2-3 months.

ESR (Erythrocyte Sedimentation Rate)

Measures systemic inflammation; elevated in autoimmune disease
ESR is an older marker of systemic inflammation, less specific than hs-CRP but still useful. Elevated ESR indicates significant inflammation. Often used alongside hs-CRP.
Target < 20 mm/hr
Suggested Action Plan

ESR improves as underlying autoimmunity is treated. Monitor ESR alongside hs-CRP to assess inflammation burden.

Thyroid Antibodies (TPO, TgAb)

Hashimoto's thyroiditis is the most common autoimmune disease; early detection enables treatment before thyroid failure
TPO (thyroid peroxidase) and TgAb (thyroglobulin) antibodies indicate autoimmune thyroid attack. Many people are antibody-positive for years before developing hypothyroidism. Early detection enables intervention.
Target Negative (< 35 IU/mL)
Suggested Action Plan

If positive, monitor TSH and free T4 regularly (every 6-12 months). Begin anti-inflammatory interventions. If thyroid function declines, thyroid hormone replacement is straightforward and highly effective.

Vitamin D

Critical for immune tolerance; deficiency promotes autoimmunity
Vitamin D is essential for T regulatory cell development, which suppresses autoimmunity. Deficiency is associated with increased autoimmune disease risk. Vitamin D optimization is part of autoimmune prevention.
Target > 50 ng/mL
Suggested Action Plan

If deficient, supplement vitamin D3 4000-5000 IU daily. Target levels of 60-80 ng/mL for immune tolerance. Retest in 8-12 weeks.

Get Comprehensive Autoimmune Screening

Test ANA, tissue-specific antibodies (TPO, TgAb especially), hs-CRP, ESR, complement levels, and vitamin D. If any antibodies are positive, continue with specific autoimmune testing (anti-dsDNA, anti-Sm, anti-SSA, anti-SSB).

Understand Antibody Positivity Without Disease

If antibodies are positive but you have no symptoms and thyroid/other organ function is normal, you are in the "preclinical autoimmunity" stage. This is the window for preventive intervention.

Adopt Anti-Inflammatory Lifestyle

Eliminate known food triggers (gluten, dairy, processed foods if sensitive). Increase omega-3 fatty acids. Exercise regularly. Manage stress through meditation or yoga. Sleep 8+ hours. These interventions reduce autoimmune activation.

Optimize Vitamin D Aggressively

Vitamin D is immunoprotective. If deficient, supplement 4000-5000 IU daily plus sun exposure. Target levels of 60-80 ng/mL specifically for immune tolerance and autoimmune prevention.

Monitor Biomarkers Every 6-12 Months

If antibodies are present, retest every 6-12 months to monitor disease trajectory. If antibody levels are rising, intensify interventions. If stable or declining, your prevention strategy is working.
